Coverage, including co-pays and deductibles for outpatient psychotherapy depends on your individual health insurance policy.  You are responsible to pay any amount your insurance does not cover so it's best to contact your company directly and we have provided a document that can help: Questions to Ask Your Insurance Company

Your individual counselor may provide you with their specific forms however, here are some intake forms they may instruct you to complete and bring to your initial interview and assessment:

I don’t have my insurance card with me, can I still receive treatment?

Your counselor may ask for your insurance information prior to your first office visit however, it's best to bring your insurance card, photo ID, copayment and any completed paperwork to your initial session.
